I/we, the parents or guardians, authorize Crossview Church to use the submitted pictures of our child(ren) for the express purpose of celebrating Child Dedication at Crossview Church and understand they will be used in the live broadcast of the service.

I/we, the parents or guardians, authorize the staff or volunteers of Crossview Church to take pictures and videos of our child(ren) for the express purpose of celebrating Child Dedication and may be posted on Crossview's social media accounts in celebration of the Child Dedication.

Children's names will not be published, nor will children or their families be tagged in any photos on social media.
